What is it about "It was a mistake that a 3MB file was sent" is difficult 
for folks to understand?

What is it everyone believes they are supposed to do about a mistake?

Additional policies do not prevent mistakes.  They just make for more 
cumbersome bureaucracy.

So what is it that folks believe is productive to do, rather than just 
being satisfying to complain about?
