[Membership-affairs] Seeking your feedback on a membership engagement survey

I also wanted to add that a question re volunteering for committees and
working groups (have you participated on x, y, or z) with some questions
re why not (intimidated, too much time, not enough background knowledge,
wrong time zone, etc) would be good.
