[NCUC-EC] membership removal process

Dear EC

It came to my attention we don't have an official membership removal
process - not in our old or new bylaws and not in our Operating
Procedures.

We are in the process of bylaws approval (likely after ICANN61)

After that, I propose we amend our Operating Procedures with the following item
