This ballot could be seriously confusing
The survey offers you options A, E, I, M, Q and U
But the pdf file has letters A – W inclusive with substantially different designs on each page.
I would re-letter pages A – D, as all A’s, E – H as all E’s, and so on.
It is of course possible that people will figure it out, as I did, but if you want a cleaner and clearer vote I would re-letter the pages

Attached please find the next round of exploration to the chosen logos. For each, they show the logo with the acronym, logo with acronym + spelled out name, logo with spelled out name and/or a variation and lastly what each would look like on a business card.
