[NCUC-EC] outreach initiatives

I've separated sections - Online/Onsite/Asynchronous and
Organizer/Co-organizer/Speaker/Participant

Did that as I think it is important to highlight members who are
taking on their shoulders to organize a whole event, no matter if
online or offline, since that is a lot of what we seem to be dealing
in the future.

Outreach initiatives which are growing are on academia and regional so
I think now the form can highlight them better

If you all find it's good to go, we can share the form w/ members and
leave it as a permanent fixture on NCUC website and/or wiki
