True, it doesn't work on my iPhone.

Rafik, possible to have the exact URL on the google doc so that we link to
it directly (rather than an Iframe on the confluence that is not rendered
on iDevices)?

If ok with NCSG, we (ncuc e-team) can also design simple form and do away
with the google docs.
